The basis of coagulopathy in COVID-19

Coagulopathy seems to be central to the worsening of COVID-19 symptoms, posing a unique challenge for the management of patients with this novel disease.
During COVID-19 pathology, depending on the severity of the condition, dysregulation has been noted in various circulating inflammatory coagulation biomarkers involved directly in clotting: P-selectin, fibrinogen, D-dimers and VWF. Depending on direction, dysregulation of these biomarkers may result in either hypercoagulation or excessive bleeding and thrombocytopenia. Haemostasis parameters including fibrinogen D-dimer and VWF levels will allow doctors to determine if their patients need either therapeutic antithrombotic prophylaxis/treatment or fibrinolytic therapy to prevent coagulopathy.
